| Separation Information

If the intention of your portfolio is to continue to employ an individual for whom you have received a separation notification email, you must initiate an Employment Transaction Form, obtain the appropriate signatures, and forward this to Human Resources 30 days prior to the end date. If the intention of your portfolio is to retain this individual as an unpaid guest or visitor, the departments listed below must be contacted individually to set up guest access as required. Specific department policy will determine relevant guest access. Failure to take these actions will affect the terminating employee in the following ways: Purchasing: (Eileen Biggins, 518-276-2724) Corporate purchasing card issued in the employee's name will be canceled. Please note that Rensselaer's Purchasing Policy prohibits unpaid guests from having access to a Corporate purchasing card. Benefits: (Christine O'Grady, 276-6302) COBRA eligibility will be determined. Benefits will end in accordance with the Summary Plan Document provisions. Payroll: (Payroll Office, 518-276-8696) Interruption in pay For fixed term employees, please note that any unused sick time and vacation time is forfeited regardless of an extension. All sick and vacation time must be used prior to the termination date listed here. If the appointment is extended, sick and vacation time will begin accruing from zero. Travel: (Payroll Office, 518-276-8696) Please submit, prior to termination, any outstanding travel expense reports Administrative System Access: (iacs-cus-sup-l@lists.rpi.edu) Access to any administrative system, including the following, will shut down:
